Steps to Refer a Patient:

1. Fill out this form to provide information regarding the client and patient. 

2. Email all patient records (including diagnostic imaging and lab work results) to palmettovetrehab@gmail.com

3. Once I receive both the referral form and records, I will reach out to the client to schedule an appointment. You can also direct the client to fill out the new client information form (under the "For Pet Owners" tab) to expedite the process.

4. Following the patient's visit, you will receive a copy of the evaluation and treatment recommendations report. I will refer patients back for all primary care.
